A Community Information Exchange (CIE) is a system that connects various partners from different
disciplines. These partners use a common language, a shared resource database, and an integrated
technology platform to improve community care planning. Tools for care planning help partners
combine data from multiple sources and send referrals both ways, creating a shared, continuous record.
With these core elements, a CIE helps communities move from reactive care to proactive, holistic, and
person-centered care. The Ventura County CIE is referred to as VCCIE.
A technology platform facilitates the integration of individual data from multiple partners’ data systems to populate an easy-to-access single, longitudinal person record, enables bi-directional closed-loop referrals, and care coordination alerts.
A standardized listing of health, human, and social services providers’ service offerings, eligibility, and intake information to efficiently match individuals with appropriate services.
The VCCIE enables a proactive system of care enabling multidisciplinary partners to work collectively and contributing to a single care plan and identify system interaction.
The use of a unified individual longitudinal record reflects shared language defining needs, risk assessments, care planning, and individual outcomes.
The VCCIE partners commit to a collective approach considering how the care they provide fits into the broader care delivery system, to better connect individuals and families to services, and share deeper levels of information.
Social determinants of health are the conditions in which people are born, grow, live, work, play, and age. These factors influence health, functioning, and quality of life. Historically, these determinants weren’t seen as key drivers of health. However, we now know that conditions tied to race, social class, and gender often lead to health inequities, significantly impacting health outcomes.
The Ventura County Community Information Exchange (VCCIE) fosters collaboration among health, human, and social service providers. Together, they deliver coordinated, person-centered care that addresses both physical health and the broader social factors affecting well-being. With tools like the Social Determinants of Health Assessment, VCCIE helps identify non-medical services tailored to meet each client’s unique needs.
