What To Expect As A VCCIE Partner

  1. Orientation: A meeting that provides an overview of the VCCIE’s purpose and commitment to the community and begins to gather information on your organization and network.​
  2. Engagement: A meeting to explore the fundamentals of onboarding to the VCCIE including Onboarding Assistance Program when applicable. ​
  3. Pre-Onboarding Assessment: Participants complete a survey to determine their integration needs ( if any) as well as existing referral partner network. ​
  4. Participation Agreement: The Participation Agreement governs the access and protection of client data. The Participation Agreement will be signed by all onboarding organizations.​
  5. Organizational Setup Assessment: Participants complete a set of questions to determine the organizational and user set up for the system. ​
  6. Client Authorization Training:  A training will be offered to support partner organization’s integration of the VCCIE’s universal client authorization form/language into the intake processes. ​
  7. User Training:  A training will be offered to demo the VCCIE system including the role based access design of the system and participants approved access based on their approved credentials.  ​
  8. Go Live: Participants who have completed the onboarding have full access to the VCCIE.

Community Partner Group Funding Opportunity

We’re excited to announce that the VCCIE is currently taking applications to fund a second cohort of Community Partner Groups! Ventura County based 501c3 social service providers who are committed to improving care coordination throughout the County are encouraged to apply.  For a successful application the following criteria need to be met:  

1) Operate as a non-profit Ventura Based 501c3 organization

2) Attend an Orientation session 

3) Attend an Engagement meeting (or listen to the recording)  

4) Submit an OPIS form as the first step to start the Participation Agreement process
