What is a CIE?
A Community Information Exchange (CIE) is an ecosystem comprised of multidisciplinary network partners that use a shared language, a resource database, and an integrated technology platform to deliver enhanced community care planning. Care planning tools enable partners to integrate data from multiple sources and make bi-directional referrals to create a shared longitudinal record.
By focusing on these core components, a CIE enables communities to shift away from a reactive approach to providing care toward proactive, holistic, person-centered care. Ventura County CIE is hereinafter referred to as VCCIE.
VCCIE System Features
Integrated Technology
A technology platform facilitates the integration of individual data from multiple partners’ data systems to populate an easy-to-access single, longitudinal person record, enables bi-directional closed-loop referrals, and care coordination alerts.
Resource Database
A standardized listing of health, human, and social services providers’ service offerings, eligibility, and intake information to efficiently match individuals with appropriate services.
Cross-System Care Planning
The VCCIE enables a proactive system of care enabling multidisciplinary partners to work collectively and contributing to a single care plan and identify system interaction.
Shared Language
The use of a unified individual longitudinal record reflects shared language defining needs, risk assessments, care planning, and individual outcomes.
Multidisciplinary Partners Network
The VCCIE partners commit to a collective approach considering how the care they provide fits into the broader care delivery system, to better connect individuals and families to services, and share deeper levels of information.
The CIE's Impact
Improved Community Wellbeing: The VCCIE's collaborative care approach, supported by cross-sector partnerships, aims to improve real-time care coordination, decrease the reliance on costly public services, and support individuals and families in achieving progress across various quality of life indicators known as social determinants of health.
Enhanced Community Care: By strengthening the technological infrastructure connecting network partners and clients, the VCCIE improves access to services for all members of the community and increases the efficiency of those services, ultimately contributing to the overall well-being of Ventura County residents.

What are the Social Determinants of Health?
Social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.
Historically, social determinants of health were not considered primary drivers of health status at either the individual or community level. But today, we understand that differential conditions, which are often closely linked to race, social class, and gender, create health inequities with impacts on health outcomes.
VCCIE Connection to Social Determinants of Health
The VCCIE harnesses the value of cross-sector collaboration that enables a network of health, human, and social service providers to deliver coordinated, person-centered care that addresses not only a person’s physical health but also the social impact of determinants of health on individual well-being.
The VCCIE is equipped with tools like it’s Social Determinants of Health Assessment, which identify services relevant to each client’s unique needs on a spectrum that includes non-medical components of health.
